Warning! You are in jeopardy of losing your health benefits!
As an In-Home Supportive Services (IHSS) Provider working for clients in Stanislaus County you are eligible for health benefits if you work 75 hours or more for three (3) consecutive months. You must continue to work 75 hours or more per month to continue with health benefits.
A check on your hours indicates that they have dropped below the required level, or you have been terminated as a provider in Stanislaus County.
- You have 45 days to increase your paid hours.
- If payroll does not show an increase in your paid hours by (DATE) your benefits will be terminated.
- You must turn your timecards in on time. Failure to do so may cause your insurance to be terminated due to low hours on the payroll report.
If you do not increase your hours, you will receive a termination notice (COBRA Notice). You will be given the opportunity to continue your health benefits at your cost. Information will be provided on how to continue your health coverage at that time.
